The easiest way to get an S+ Rank is to do a fresh playthrough with Special Items unlocked. This means that you'll need to finish the main story in different difficulties first (with any rank) to have them available for purchase.
Special items like Ashley's Armor and Chicago Sweeper can be used in a fresh save after they're obtained, and will not lock you out of an S+ Rank.

Using bonus weapons does not prevent you from earning S+ Rank. Weapons such as the Chicago Sweeper, Handcannon, and Primal Knife can all be used during a fresh New Game run.
Likewise, accessories earned from previous playthroughs, including the Chicken Hat, Deer Antlers, and Gas Mask, are also allowed and will not affect your final rank.

If you've unlocked the Chicago Sweeper, complete enough Merchant Requests in the Village to collect 30 Spinels. Once you reach the first Merchant in the Castle, exchange them for an Exclusive Upgrade Ticket and use it on the Chicago Sweeper to unlock Infinite Ammo.
This strategy makes the remainder of the playthrough significantly easier, allowing you to defeat most enemies without worrying about conserving ammunition.

The only requirement for earning S+ Rank is finishing the game within the required time. Avoid spending extra time exploring areas for optional Collectibles, Hidden Treasures, or other completion objectives that are not on your route. Instead, focus on reaching the next objective as quickly as possible.

This is self-explanatory since boss fights tend to be lengthy especially when your weapons are merely peppering your enemy instead of blasting holes through them.
The in-game timer stops while you're in the pause menu, inventory, map, Merchant shop, Typewriter menu, and Photo Mode. Use these breaks to organize your inventory, craft ammunition, or plan your next objective without adding to your total playtime.
Sometimes the best way to win the fight is to win it from the shadows. This will allow you to sneak past enemies and reach checkpoints earlier since you won't have to deal with every enemy head-on.
When stealth is not an option, you have a higher chance of getting S+ rank when you run away from your enemies instead of standing your ground to fight them all. Of course this will not always be the case as some stages will require you to stand your ground.
In higher difficulties such as Professional, autosaves are disabled. If you die, you would have to begin again from your last save. This would waste precious seconds as you will have to run past everything again.
| Rank | Assisted | Standard | Hardcore | Professional |
|---|---|---|---|---|
| S+ | 04:00:00 | 05:00:00 | 05:30:00 | 05:30:00 15 Saves Only |
| S | 04:00:00 | 05:00:00 | 05:30:00 | 05:30:00 |
| A | 15:00:00 | 12:00:00 | 10:00:00 | 07:00:00 |
Each difficulty will have a different cutoff time for each playthrough length. It can be assumed that anything more than the playtimes exceeding those along the A row would then be classified as B.
